Primary Care Nurse Practitioner

The Advanced Practice Clinician (APC) plays a vital role in our collaborative team at Southeast Primary Care Partners, working alongside Family Practice and Internal Medicine Physicians, RN Case Managers, and clinic staff to provide comprehensive, coordinated care to our patients.
The APC primarily manages patient populations in conjunction with primary care physicians and specialists, providing both chronic and acute care in outpatient and inpatient settings. As an extension of the primary care physician, the APC will diagnose and treat various conditions, including diabetes, hypertension, and kidney disease, as well as minor injuries and acute medical conditions.
The APC will also order and interpret diagnostic lab work, x-rays, and EKGs, prescribe medications and other treatments as needed, and counsel patients on disease conditions to ensure they are educated on their treatment and expected outcomes.
As a key member of our team, the APC will work collaboratively to provide quality, evidence-based care to our patients and perform other related duties as assigned.
Essential Job Functions- Provide care consistent with licensure and/or certification.
- Diagnose and treat acute and chronic conditions, including diabetes, hypertension, and kidney disease.
- Order and interpret diagnostic lab work, x-rays, and EKGs.
- Prescribe medications and other treatments as needed.
- Counsel patients on disease conditions.
- Work collaboratively with the team to provide quality, evidence-based care.
- Perform other related duties as assigned.
Minimum Required Education, Experience & Skills
- Graduate credit for PA/NP Program.
- Current Licensure through the Georgia Board.
- Current prescriptive authority from the Georgia Board.
