As an LPN, you will play a vital role in providing high-quality care to patients in their homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Provide direct nursing care to home health patients, following established care plans and protocols.
  • Conduct regular home visits to assess patient needs and provide education and support to patients and their families.
  • Collaborate with the interdisciplinary team to develop and update patient care plans, ensuring that patients receive comprehensive and coordinated care.
  • Document patient interactions, interventions, and outcomes in a timely and accurate manner.
  • Participate in performance improvement activities to enhance the quality of care and services provided.
Requirements
  • Basic Cardiac Life Support (BCLS) certification
  • Valid Pennsylvania LPN licensure
  • High school diploma or equivalent
  • Graduate of an NLN-accredited licensed practical nursing program
