Job Summary

We are seeking a skilled Speech Language Pathologist to join our team at Revival Home Health & Hospice. As a Home Health Speech Language Pathologist, you will work with patients in their homes, providing individualized care and support to help them achieve their maximum level of speech, language, cognitive, and swallowing abilities.

Key Responsibilities
  • Perform comprehensive evaluations and develop treatment plans to address patients' speech, language, cognitive, and swallowing disorders
  • Collaborate with interdisciplinary teams to provide coordinated care and support
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
  • Maintain accurate and timely documentation of patient progress and treatment plans
  • Participate in ongoing education and professional development to stay current with best practices and industry developments
Requirements
  • Master's degree in Speech Language Pathology
  • Current licensure in Speech Language Pathology and American Heart Association CPR certification
  • At least one year of experience in speech pathology, with home health experience preferred
  • Excellent observation, verbal, and written communication skills
  • Problem-solving skills and basic math skills
