About the Role:- Lead a team of care coordinators to achieve excellence in daily operations, focusing on quality, compliance, and member outcomes.
- Provide coaching and support to the care management team, addressing problems, resolving issues, and navigating regulatory and service-related changes.
- Manage and coordinate the activities of the VNS Health Plans Care Coordination team to ensure member care coordination is consistent with Enterprise mission and strategic goals.
- Partner across the organization to achieve goals and provide comprehensive, community-based care services to our members.
