Job Title: Mental Health Juvenile Justice Liaison

We are seeking a highly motivated and experienced Mental Health Juvenile Justice Liaison to join our team at SGA Youth and Family Services. The successful candidate will be responsible for providing mental health services to at-risk youth referred from Cook County Juvenile Probation, Illinois Department of Juvenile Justice, and other local partners.

Key Responsibilities:
  • Provide case management services, including individual needs assessments, goal setting, and linkage with community agencies.
  • Maintain an active caseload of 40-45 youth per quarter, with a focus on providing high-quality services and achieving positive outcomes.
  • Evaluate at-risk youth using the Childhood Severity of Psychiatric Illness (CSPI) assessment tool and develop Action Plans to address identified needs.
  • Link youth with community-based mental health providers and coordinate follow-up assessments to ensure continuity of care.
  • Develop and maintain partnerships with local community resources to strengthen referral base and improve services.
  • Transport youth to and from program-related activities, as needed.
  • Implement workshops and other program activities to engage youth and promote positive outcomes.
  • Enter case notes, assessments, and case plans in a timely manner and respond to email communication.
  • Complete required reports and data materials for SGA and DHS/DMH.
Requirements:
  • Bachelor's Degree in psychology, social work, or a related field.
  • One to three years of experience working with at-risk youth, preferably in a juvenile justice setting.
  • Bilingual/bicultural in Spanish/English, preferred.
  • Knowledge of the Cook County Juvenile Justice System and experience working in high-risk neighborhoods.
  • Ability to communicate effectively with diverse families, professionals, and community groups.
Physical Requirements:
  • Ability to remain stationary for 50% of the time.
  • Ability to frequently move inside the office and transport oneself to and from home visits.
  • Occasional ascension/descent of stairs and ability to lift up to 25 lbs.
