About the Role

In this exciting role as a Case Manager RN - Cardiovascular, you will have the opportunity to make a real difference in the lives of our patients by providing exceptional care coordination and navigation throughout their treatment journey.

You will work closely with multidisciplinary teams to ensure seamless transitions between pre-surgery and post-surgery treatments, leveraging your expertise to optimize patient outcomes.

Key Responsibilities
  • Pre-Surgery Planning: Collaborate with patients, families, and healthcare providers to develop personalized pre-surgery care plans.
  • Clinical Coordination: Coordinate clinical activities, including surgical scheduling, lab tests, and medication management.
  • Post-Surgery Support: Provide ongoing support and education to patients and families after surgery, ensuring a smooth transition to post-discharge care.
  • Communication: Serve as a liaison between patients, families, and healthcare providers, facilitating open communication and addressing concerns.
Requirements
  • Education and Licensure: Bachelor's degree in nursing or health-related field, or four years of experience in a directly related field. Registered Nurse License (California) required.
  • Experience: Minimum three years of patient care experience required.
Preferred Qualifications
  • Case Management Experience: Minimum six months of previous case management experience preferred.
  • Care Coordination Experience: Minimum one year of care coordination or cardiovascular patient care services experience preferred.
  • Utilization Management: Experience in utilization management, discharge planning, or transfer coordination.
Compensation and Benefits

The estimated annual salary for this role is $103,500, with opportunities for professional growth and development within Kaiser Permanente.
