Occupational Therapist

We are seeking a skilled and compassionate Home Health Occupational Therapist to join our team at LHC Group. As an Occupational Therapist with our organization, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

The Home Health Occupational Therapist will provide high-quality, patient-centered care to individuals in their homes, promoting independence, mobility, and overall well-being. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with healthcare teams to develop and implement effective treatment plans.

• Evaluate patients' functional status and occupational therapy needs, developing personalized treatment plans to address their unique challenges and goals.
• Provide evidence-based occupational therapy interventions, including exercises, activities, and education, to promote patient progress and independence.
• Collaborate with healthcare teams, including physicians, nurses, and other therapists, to ensure comprehensive and coordinated care.
• Document patient progress and outcomes, maintaining accurate and detailed records of treatment plans and patient responses.
• Participate in staff conferences and committees to share knowledge, best practices, and ideas for improving patient care and outcomes.

• Current Occupational Therapy licensure in the state of New York.
• Current CPR certification.
• Current driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation.
