Occupational Therapist

We are seeking a skilled Occupational Therapist to join our team at Willcare, a part of LHC Group, in Wellsville, NY. • Provides occupational therapy services to patients in their homes, using evidence-based techniques and in coordination with other healthcare team members.
• Evaluates patients' functional status, occupational therapy needs, and develops individualized plans of care.
• Consults with physicians and other healthcare professionals to ensure comprehensive care.
• Observes, records, and reports patients' responses to treatment and changes in their condition.
• Instructs patients and their families/caregivers on functional limitations, progress towards goals, and plan of care.
• Visits patients in their homes, completes progress notes, and submits reports as required.
• Participates in staff conferences and committees as requested.
• May supervise home health aides and occupational therapy assistants as per LHC policy and state regulations.

• Current Occupational Therapy licensure in the state of New York.
• Current CPR certification.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
