Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home. Responsibilities
  • Provide quality patient care and perform technical skilled care in the patient's home.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor patients' progress and report any changes or concerns to the healthcare team.
  • Provide education and support to patients and their families on disease management and self-care.
Requirements
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
  • Additional State Specific Requirements: TN - No additional state specific requirements.
