The Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Key responsibilities include:

  • conducting initial and comprehensive nursing evaluations to determine primary focus of care
  • developing and implementing individualized plans of care in collaboration with the patient, family, and healthcare team
  • ensuring patients meet home health eligibility and medical necessity guidelines
  • maintaining accurate and timely documentation of patient care and progress

Requirements for this role include:

  • current RN licensure in the state of practice
  • current CPR certification
  • current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
