Job Summary

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team. As an LVN at Sunnyside Nursing and Post-Acute Care, you will play a vital role in providing exceptional care to our residents, while collaborating with our interdisciplinary team to ensure seamless care transitions.

Key Responsibilities
  • Deliver high-quality care and customer service to residents, families, and visitors
  • Administer medications in accordance with physician orders and maintain accurate records
  • Document care and treatment under Nurses' notes, consistent with the plan of care
  • Verify resident care plans accurately reflect goals, problems, approaches, and revisions based on resident needs
  • Complete and file required recordkeeping forms/charts upon admission, transfer, and/or discharge
  • Chart reports of accidents/incidents and changes of conditions involving residents, and complete facility reports as necessary
  • Verify adequate supply of floor stock medications, supplies, and equipment, and report needs to the Nurse Supervisor
  • Order prescribed medications, supplies, and equipment as necessary, and in accordance with established policies
  • Supervise Certified Nursing Assistants to ensure high-quality care and adherence to job duties
Requirements
  • Current and active California LVN License
  • Authorization to work in the United States
