Job Summary:

The Licensed Practical Nurse (LPN) - Medical Support Specialist provides direct medical support and education to individuals in assigned programs at Seven Hills Foundation. This role involves assessing and coordinating healthcare services, advocating for medical needs, and communicating with healthcare professionals.

Key Responsibilities:

  • Assess and Coordinate Healthcare Services: Evaluate the medical needs of program participants and coordinate healthcare services to ensure optimal health outcomes.
  • Advocate for Medical Needs: Advocate for the medical needs of program participants, focusing on individualized care and support.
  • Communicate with Healthcare Professionals: Communicate with healthcare professionals, including physicians, residential managers, and healthcare directors, to ensure seamless care coordination.
  • Document Medical Interventions: Document all medical interventions and changes affecting program participants in appropriate areas, including TIER, progress notes, and staff communication logs.
  • Review ISPs and Maintain Health Records: Review Individualized Service Plans (ISPs) for current knowledge of programming and maintain accurate and up-to-date health records according to DDS standards.
  • Ensure Safety and Quality Care: Monitor for the observance of safety protocols and ensure that information is communicated to staff regarding recommendations and treatments.

Qualifications:

  • LPN or RN License: Current LPN or RN license in Massachusetts.
  • Experience Working with Individuals with MR/DD: One year of experience working with individuals with mental retardation/developmental disabilities.
  • Valid Driver's License: Valid driver's license required.
