Job Title: Licensed Practical NurseJob Summary:

Bridgewell is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN, you will provide direct care and health services to individuals with disabilities living in a residential program. Your expertise will be essential in ensuring the health and well-being of our residents, and you will work collaboratively with our healthcare team to provide comprehensive care.

Responsibilities:
  1. Provide direct specialized nursing and day-to-day care to individuals with disabilities.
  2. Document health care services and maintain accurate medical records.
  3. Collaborate with the residential manager, registered nurse, and Director of Health Care to ensure seamless care delivery.
  4. Act as a liaison with community health care providers and monitor medical records.
  5. Administer medications and maintain knowledge of the Department of Public Health Medication Administration Program (MAP) policies and procedures.
Requirements:
  1. Graduation from an accredited and approved licensed practical nurse program.
  2. Current Massachusetts Nursing license.
  3. Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred.
