We are seeking a highly skilled Behavioral Health Therapist to join our team at Mid Michigan Health. As a key member of our primary care treatment team, you will play a vital role in identifying, assessing, and managing patients with medical and behavioral health problems.
Responsibilities- Assessment and Triage: Conduct thorough assessments of patients referred by primary care providers, identifying at-risk patients and intervening to prevent further psychological or physical deterioration.
- Clinical Intervention: Provide brief behavioral interventions using evidence-based techniques, such as behavioral activation, problem-solving treatment, and motivational interviewing.
- Documentation and Follow-up: Systematically document treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
- Program Development: Assist in developing programs to address common health issues, such as pain management, diabetes, and weight loss.
- Collaboration and Communication: Work closely with primary care providers, psychiatrists, and other specialists to ensure seamless care coordination and effective treatment plans.
- Education: Master's degree in Social Work (MSW) or a related field.
- Licensure: Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan.
- Experience: At least one year of experience in a primary care setting or a related field.
