**Position Details:**

* Part Time, non-exempt
* Open Positions: Part Time
* Schedule: 6:45PM-7:15AM; every Wednesday & Thursday
* Location: Gelhard House; PC Memory Support Household; up to 13 residents

**Compensation and Benefits:**

* Base Pay: Determined based upon years licensed and experience
* Shift/Weekend Differentials: $1.50 additional from 3:00pm-11:00pm, $0.75 additional from 11:00pm-7:00am, $1.25 additional on Weekends (Saturday & Sunday)
* Benefits: Medical, Dental & Vision - 100% covered by Employer, Short Term Disability - 100% covered by Employer, Group Term Life Insurance - up to $50, % covered by Employer, 401k or Roth IRA; max employer match is 4.5%, Accrual Banks: Vacation & Sick Time, Perfect Attendance Bonus Program, Referral Bonus Program, Education Assistance Program, Funeral Leave, Holiday pay; including your Birthday as a recognized Holiday

**Responsibilities:**

* Establish respectful, warm relationship with residents, perform rounds
* Admission of residents
* Develop/update of resident pre-screen, assessment, and support plans
* Communicate, as necessary, with physicians and resident families
* Staff supervision and coaching
* Assist with meeting resident activities of daily living as needed/indicated
* Assist with medication administration as needed
* Participate in weekly PCH Supervisory meetings
* Assist with emergency response campus-wide, as needed
* Attend all required staff meetings and complete monthly education on Relias by dates due
* Adhere to all agency policies and procedures, federal, state, and local regulations, and OSHA safety standards

**Requirements:**

* Currently licensed in the Commonwealth of PA as a Licensed Practical Nurse
* Minimum of three years as an LPN
* Minimum of three years of supervisory experience
* Experienced in developing support plans for elders
* Familiarity with Personal Care Home Regulations
* Hands-on experience in ADL assistance
* CPR/AED certified or able to be certified
* Written & verbal skills to communicate with client, families, and staff
