We are seeking a highly skilled Oncology Nurse Navigator to join our team at American Oncology Network, Inc. The successful candidate will be responsible for providing individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers.
Key Responsibilities- Care Coordination: Collaborate with healthcare providers to coordinate the care of patients with a past, current, or potential diagnosis of cancer, adhering to NCCN guidelines.
- Patient Education: Provide education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer care continuum.
- Communication: Ensure effective communication and premier customer service with internal and external customers, including relevant updates and quarterly patient services data to the director.
- Clinical Trial Access: Collaborate with healthcare providers on potential trials, participate in screening and consenting patients for clinical trials, and assist with startup of trials.
- Teamwork: Maintain an in-depth understanding of program requirements, assess patient needs for resources, and participate in outreach programs.
- Education: Minimum of Associate degree in Nursing, Bachelor of Science in Nursing preferred.
- Experience: Minimum of five (5) years' experience in Oncology nursing, clinical trial experience preferred.
- Certifications: Registered Nurse required, OCN certification required, valid Driver's license, current BLS, and ONN certification preferred.
