Home Health Care Professional

We offer a competitive salary of $32,000 - $40,000 per year, depending on experience, to support your career growth and well-being.
As a Home Health Aide, you will work closely with our multidisciplinary team to deliver patient-centered care that prioritizes dignity, respect, and safety. Your primary responsibilities will include:
- Assisting patients with daily living activities, such as bathing, dressing, and grooming.
- Providing emotional support and companionship to patients and their families.
- Maintaining a clean and safe environment by completing light housekeeping tasks and laundry.
- Preparing meals and snacks according to patient instructions.
- Accompanying patients to medical appointments and therapies.
- Administering medications and performing routine medical tasks, such as taking vital signs.
- Documenting patient progress and reporting any changes or concerns to the healthcare team.
- A high school diploma or equivalent.
- Current certification as a Home Health Aide (HHA) in New York State.
- Ability to lift 20 pounds and perform physical demands of the job.
- Reliable transportation and a valid driver's license.
- Excellent communication and interpersonal skills.
