**Job Title:** Registered Nurse Care Coordinator

Job Overview

We are seeking a highly skilled Registered Nurse Care Coordinator to join our team at Community Care of WNY Inc. The successful candidate will be responsible for coordinating case management, supervising services staff, and providing skilled nursing services.

**Responsibilities:

  • Provide orientation, annual physical assessments, in-service education, supervision, and evaluation to paraprofessional services staff and volunteers as assigned.
  • Supervise and evaluate paraprofessional performance in the home, reporting unresolved performance issues to the Senior RN Care Coordinator.
  • Ensure delivery of services within agency-defined parameters and patient plans of care.
  • Track and monitor required supervisory or assessment visits for assigned patients and staff.
  • Develop and update patient plans of care and documentation.
  • Provide direct hands-on nursing care to patients as defined in the Interdisciplinary Team plan of care.
  • Conduct initial assessments of new referrals and serve as a member of the Interdisciplinary Team.
  • Assist in volunteer or staff training and inservices as needed.

**Requirements:

  • Current license to practice nursing as a registered professional nurse in the State of New York.
  • A minimum of two years full-time or equivalent recent applicable nursing experience.
  • Ability to lift 20 pounds, perform one-person transfers, and reposition bedbound patients.
  • Effective oral and written communication skills.
  • BLS certification.
  • Acceptable driver's license and automobile with current insurance.
  • Computer literacy and ability to work in an Electronic Medical Record.
