Job Description:
As an LPN with IntelyCare, you'll play a vital role in providing high-quality care to patients in post-acute facilities. You'll have the autonomy to create your own schedule, choosing from a variety of shifts and facilities. Key Responsibilities:
• Deliver patient-centered care in accordance with resident care plans
• Monitor and report any changes in residents' condition
• Administer medications and maintain accurate medical records
• Collaborate with other healthcare professionals to ensure seamless care delivery
• Participate in ongoing education and professional development opportunities

Qualifications and Skills:
• Current Massachusetts state license to practice as a LPN
• At least 6 months of experience in a similar role
• Strong communication and teamwork skills
• Ability to lift a minimum of 50 lbs
• Willingness to work flexible hours, including weekends and holidays
