BAYADA Home Health Care is seeking a compassionate and skilled Home Health Physical Therapist to join our team in East Norriton, Pennsylvania. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality patient care in the comfort of our patients' own homes. As a Home Health Physical Therapist, you will be responsible for assessing patients' physical abilities, developing and implementing individualized care plans, and providing manual therapeutic exercises, gait training, and other interventions as needed.

Responsibilities
  • Assess patients' physical abilities and develop individualized care plans
  • Provide manual therapeutic exercises, gait training, and other interventions as needed
  • Continually assess and revise care plans as necessary
  • Collaborate with nursing staff to ensure comprehensive patient care
Requirements

To be successful as a Home Health Physical Therapist, you will need:

  • A current and active license to practice as a Physical Therapist in the state of Pennsylvania
  • Previous home healthcare experience is welcomed, but not required
  • Ability to travel to cases as assigned
  • A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication
