Role Summary:

  • The Speech-Language Pathologist is tasked with aiding individuals with medical and functional challenges to achieve and regain optimal functionality through a patient-focused care model. This professional collaborates with a multidisciplinary team to meet the goals outlined in each patient’s personalized care plan, developed by the rehabilitation physician, and adjusts strategies based on patient progress.

Key Responsibilities:

  • Delivering professional speech therapy services.
  • Upholding confidentiality standards.
  • Integrating ethical principles into practice, advocating for patient rights and reporting any malpractice.
  • Evaluating personal practice against professional standards and regulations.
  • Providing health education tailored to the developmental levels and cultural backgrounds of patients.
  • Ensuring continuity of care post-discharge by designing home exercise programs and recommending follow-up services.
  • Enhancing the quality of practice through participation in performance improvement initiatives.
  • Demonstrating a commitment to continuous learning and professional development.
  • Meeting the physical, psychological, and social needs of patients across various age groups.
  • Participating in unit compliance and undertaking additional duties as assigned.

Care Delivery:

  • Conducting comprehensive assessments and documenting relevant health data.
  • Utilizing evidence-based assessment techniques to provide holistic patient care.
  • Analyzing assessment data to identify diagnoses or issues.
  • Developing individualized care plans in collaboration with patients and families.
  • Identifying measurable outcomes based on patient-centered goals.
  • Coordinating the implementation of care plans and documenting interventions.
  • Evaluating patient progress and making necessary adjustments to care plans.
  • Ensuring adherence to patient safety and quality standards.
  • Actively engaging in multidisciplinary team discussions and discharge planning.

Leadership and Research:

  • Exhibiting leadership qualities by participating in committees and community outreach.
  • Incorporating research findings into practice to enhance patient care.
  • Utilizing resources effectively to meet patient needs.

Team Collaboration:

  • Contributing to the professional growth of colleagues through knowledge sharing.
  • Delegating appropriate tasks to assistants while ensuring quality care.
  • Collaborating with team members to set wellness-oriented goals for patients.
  • Engaging in interdisciplinary team processes to optimize patient recovery.

Qualifications:

  • Required: Licensed Speech Language Pathologist.
  • Required: Basic Cardiac Life Support certification.
  • Education: Master’s degree in Speech Therapy.
  • Preferred: Two to four years of experience as a Speech Therapist.
