Job Summary
We are seeking a compassionate Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in coordinating patient care, collaborating with healthcare teams, and ensuring seamless transitions between care settings.
Estimated Salary: $65,000 - $85,000 per year
Job Description
In this role, you will be responsible for assessing patient needs, identifying outcomes, and developing plans of care in collaboration with healthcare teams. You will also maintain education and competencies to remain proficient in providing high-quality patient care practice and skills.
Required Qualifications
Associate's or Vocational degree required; 2 or more years of RN experience preferred. Preferred qualifications include Bachelor's degree, Case Management experience, and certifications such as Certified Hospice and Palliative Nursing (CHPN) or Nursing Case Management Certification (CMGT-BC).
Physical Demands
This role requires medium work, with lifting weights up to 32 lbs. occasionally and up to 25 lbs. frequently. We are committed to providing a safe working environment and adhere to our Allina Safe Patient Moving Policy.
