We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Home Health Care, Inc.

As a key member of our care team, you will provide high-quality patient care in the comfort of clients' homes. This is an exciting opportunity to make a meaningful difference in the lives of individuals and families in need of healthcare support.

Home Health Care, Inc. is a locally-owned and Medicare-certified home health agency with over 30 years of experience in providing quality health care services to residents of 21 Minnesota counties.

We value self-driven individuals passionate about delivering exceptional patient care and welcome applications from experienced LPNs looking to make a difference in their community.

Responsibilities:

  • Provide patient care according to individualized care plans and needs
  • Perform wound care treatments and assist clients with medication management
  • Teach clients and/or family members about care and support skills
  • Communicate regularly with RN Case Managers, Supervisors, and the Care Team

Requirements:

  • Licensed as a Practical Nurse / LPN in the state of Minnesota
  • Reliable vehicle with current insurance and a valid Driver's License required
  • Demonstrated ability to work autonomously and as part of a team
  • Prior Nursing experience in a TCU, Hospice, or Home Health setting preferred
  • Experience with Electronic Medical Records (EMR) software is an asset
