Career Opportunity:

We are seeking an experienced and caring LPN to join our team as a Nurse Care Specialist. This role involves providing patient care according to client's care plans and needs, performing wound care treatments, and teaching clients and their families about care and support skills.

Responsibilities:

  • ">
  • Provide patient care according to client's care plan and needs">
  • Perform wound care treatments and assist clients with medication management">
  • Teach the client and/or family members about care and support skills">
  • Communicate regularly with RN Case Managers, Supervisors, and the Care Team">
  • Qualifications:">
  1. ">
  2. Licensed as a Practical Nurse / LPN in the state of Minnesota">
  3. Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License">
  4. Demonstrated ability to work autonomously">
  5. Prior Nursing experience in a TCU, Hospice, or Home Health setting">
  6. Experience with Electronic Medical Records (EMR) software is preferred">

Salary:

The estimated salary for this position is $32 - $33 per visit.
