Job Title: Baylor Registered Nurse

Revival Home Health & Hospice is seeking a dedicated and compassionate Registered Nurse to join our team in Annandale, VA. As a Home Health RN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

About the Role:
  • Provide safe and effective patient care, including medication management and wound care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Conduct regular assessments and evaluations to ensure patient progress and adjust care plans as needed.
Responsibilities:
  • Attend and participate in weekly team meetings to discuss patient progress and address any concerns.
  • Provide education and support to patients and their families on disease management and self-care techniques.
  • Monitor and report any changes in patient condition to the physician and other healthcare team members.
  • Implement and maintain accurate and up-to-date patient records.
Requirements:
  • Current Registered Nurse licensure in the state of Virginia.
  • American Heart Association CPR certification.
  • One year of home health experience preferred.
  • Excellent communication and interpersonal skills.
