We are seeking a skilled Hospice Social Worker to join our team at ViaQuest Hospice. As a Hospice Social Worker, you will play a vital role in providing comprehensive care to our patients and their loved ones.
Key Responsibilities- Conduct thorough patient assessments to identify their needs and develop personalized care plans.
- Provide regular patient visits to ensure their physical, emotional, and mental well-being.
- Offer short-term crisis intervention and individual or family counseling as needed.
- Utilize our electronic medical record system, Brightree, to document patient information.
- Serve as a patient and family advocate, ensuring their needs are met and their voices are heard.
- Masters degree in Social Work preferred, but a Bachelors in Social Work is also accepted.
- Experience in Medical Social Work is preferred, as is experience within the Hospice field.
- Valid driver's license, car insurance, and reliable transportation are required.
- Ability to travel throughout the assigned service area is necessary.
- Comprehensive training, including patient visits with a preceptor and Brightree and Elsevier training.
