Job Title: RN Homecare Specialist

Cleveland Clinic is seeking a skilled RN Homecare Specialist to join our team. As a Homecare Registered Nurse, you will provide high-quality nursing care to patients in the comfort of their own homes.

Job Summary:

The RN Homecare Specialist will be responsible for providing direct nursing care to patients in their homes, assessing patient needs, and implementing and monitoring patient care plans. The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work independently.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, including assessments, treatments, and education.
  • Implement and monitor patient care plans established by the Care Coordinator.
  • Participate in a team-based care model led by the Care Coordinator.
  • Perform accurate and complete starts of care, resumptions, recertifications, and routine visits as requested and within specified time frames.
  • Perform medication reconciliation at each visit.
  • Assess patients for safety risks, abuse, and neglect.
  • Maintain complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions.
  • Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed.
  • Communicate any patient's fall out/missed visits to the Care Coordinator immediately.
  • Participate in and support quality improvement projects and participate in department committees as assigned.
Requirements:
  • Graduate from an accredited school of professional nursing.
  • Current state licensure as a Registered Nurse (RN).
  • One year of current clinical experience as an RN.
  • One year of experience in an acute medical/surgical or homecare setting.
  • Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
Preferred Qualifications:
  • Bachelor of science in nursing (BSN).

Physical Requirements:

  • Requires full range of motion, manual and finger dexterity, and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Requires corrected hearing and vision to normal range.
  • Requires some exposure to communicable disease or bodily fluids.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work.
  • Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
  • Must be able to read, write, and communicate with patients.

  • Follows Standard Precautions using personal protective equipment as required.
