Pinnacle Home Care Inc. is seeking a dedicated and compassionate Speech Language Pathologist to join our team of skilled home healthcare providers. As a Speech Language Pathologist, you will provide high-quality Speech Therapy services to patients in their homes, working closely with our interdisciplinary team to deliver exceptional patient care.

Responsibilities:

  • Evaluate patients' level of functioning and hearing, recommending mechanisms to enhance their hearing ability.
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement Speech Therapy treatment plans in consultation with physicians and revise them as necessary.
  • Assist in the development and implementation of interdisciplinary client care plans as they relate to Speech Therapy.
  • Communicate with other team members and instruct them in speech pathology techniques when appropriate.
  • Teach, supervise, and counsel patients and their families in the total Speech Therapy program and other related problems.
  • Record all evaluation data, treatments, and patients' responses to therapeutic intervention in a timely manner.
  • Supervise Home Health Aides every 14 days when Speech Therapy is the only professional discipline involved in patient care.
  • Record and report patients' reactions to treatment and any changes in their condition to physicians.
  • Participate in the development of periodic revisions to physicians' plans of treatment, keeping them informed of patients' status and obtaining additional orders as necessary.
  • Evaluate the effectiveness of Speech Therapy intervention and update client care plans as needed.

Qualifications:

  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, and bending; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
