Position Overview:
Role Summary:
As a Primary Care Physician within the Moses/Weitzman Health System, your responsibilities will include:
1. Delivering comprehensive medical services primarily focused on underserved communities.
2. Collaborating with a robust clinical support team in a forward-thinking practice environment that utilizes Advanced Access scheduling.
3. Engaging in a multi-disciplinary approach that integrates both individual and population health management.
Qualifications for Physicians:
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited medical institution, along with completion of a recognized residency program in family medicine.
- Board certification is required within 24 months of starting your role and must hold or be eligible for licensure in the State of Connecticut.
- Proficiency in both English and Spanish is preferred.
- Nurse Practitioners (NPs) are also welcome to apply for this role.
Qualifications for APRNs:
- Must be a graduate of an accredited Master of Science in Nursing program with a specialization as a Family Nurse Practitioner and possess ANA certification.
- State of Connecticut APRN and RN licensure is required within 12 months.
- Bilingual skills in English and Spanish are preferred.
Required Certifications:
- Medical licensure in the State of Connecticut.
Qualifications for Physician Assistants:
- Must have graduated from an accredited Master's program in Physician Assistant studies.
- Must maintain appropriate board certification, licensure, and be active in professional organizations.
