Job Description:

Summary

This position is responsible for providing comprehensive care to assigned patients, addressing their physical, psychological, social, cultural, spiritual, and developmental needs. The successful candidate will function as a contributing member of the healthcare team, working collaboratively with other professionals to deliver optimal patient outcomes.

  • Assists the team leader in assessing the status of patients with simple health problems on admissions and on a continuing basis, reporting changes in health status to the RN and/or Physician.
  • Contributes information to the plan of care.
  • Implements the plan of care, under the direction of the RN and/or Physician and within the scope of practice, to meet the basic needs of assigned patients and families.
  • Documents data for assessments, interventions, and effectiveness of care provided in the patient's medical record. Findings are reported to the RN and/or Physician.
  • Participates in patient education activities as directed by the RN and/or Physician.
  • Assists in the preparation of the patient and family for discharge under the direction of the RN and/or Physician.
  • Administers medications to assigned patients in accordance with clinical specific standards.
  • Responds to emergencies.
  • Identifies safety hazards and reports them.
  • Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the age-specific and developmental needs of patients served by the department.
  • Appropriately adapts assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, spiritual, age-specific, and other developmental needs of each patient served.
  • Demonstrates adherence to the CORE values of Christus Health.
  • Performs other duties as assigned.
Requirements
  • Education: High School Diploma or GED required; Graduate from an accredited school of vocational nursing.
  • Experience: Experience in a clinical setting preferred.
  • Licenses, Certifications, or Registrations: Licensed as a LVN in Texas; BLS Certification required.
