Clinical Team Leader Home Health Specialist

ResponsibilitiesWe're seeking a dynamic RN to bridge their career from the field to the office, providing patient care while influencing care delivery for all patients. This role is ideal for someone looking to take the next step in their career into management.
The RN Team Leader contributes to the company's success by effectively facilitating relationships between physicians, referral sources, patients, caregivers, and employees. Multiple incentive bonus opportunities are available.
Key Responsibilities:- Encourage, mentor, and inspire others to provide A Better Way To Care
- Provide oversight of patient care, assist with case conference, review, and approve orders
- Assist the Branch Director in providing ongoing education and training to all branch clinicians to ensure understanding of documentation requirements to meet regulatory standards
- Execute patient visits, including OASIS assessments, and participate in on-call rotation as needed

QualificationsWe're looking for a candidate with a graduate degree from an accredited school of professional nursing, licensed in the state of employment, and a minimum of two years of nursing experience in medical-surgical and/or subacute settings. The ideal candidate should have a good understanding of Federal, State, and local laws/regulatory guidelines governing the operation of a Home Health agency.
Skilled competency with technology, including general office equipment and EMR platforms, is required. A valid state driver's license and automobile liability insurance are also necessary. Reliable transportation is required to perform visits in patients' homes.
