Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As a LPN Home Care Specialist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide direct nursing care to patients in their homes, as prescribed by the physician.
  • Record clinical and progress notes, and coordinate other patient care activities to provide the highest quality of patient care.
  • Work under the direct supervision of the Registered Nurse and/or designated supervisor.
  • Ensure confidentiality of patient health information, and maintain accurate and up-to-date records.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience preferred.
