Hospice Spiritual Care Specialist

At Heartland Hospice, we believe that every day is an opportunity to make care more personal and life more comfortable. As a Hospice Chaplain, you will play a vital role in providing spiritual support to our patients and their families.
Key Responsibilities:- Provide spiritual services to hospice patients and families, including counseling, prayer, and ministry.
- Develop and implement the interdisciplinary care plan, working closely with the healthcare team.
- Visit patients and families to assess their spiritual needs and provide direct support.
- Assist the interdisciplinary team in understanding the significant spiritual concerns related to patient and family response to dying.
- Serve as a liaison and resource for community spiritual leaders in dealing with hospice patients and families.
- Bachelor's or Master's degree in theology, religion, human services, counseling, psychology, or sociology.
- One unit of Association of Clinical Pastoral Education, Inc. (ACPE)-accredited CPE.
- Two years of active work in spiritual care ministry or program, with demonstrated ability to work with patients, families, and community spiritual leaders of various denominations and beliefs.
