About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Heartland Hospice. As a Hospice Care Specialist, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.

Key Responsibilities
  • Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming.
  • Assist patients with mobility and ambulation, including the safe use of walkers or wheelchairs.
  • Prepare and serve light meals and snacks, as well as assist with feeding patients when needed.
  • Conduct light housekeeping tasks, such as emptying trash and changing linens.
  • Report any changes in patient condition or home situation to the Registered Nurse and interdisciplinary team.
  • Document patient care and visit itineraries accurately and in a timely manner.
Requirements
  • Certified Nursing Assistant (CNA) license.
  • At least six months of patient care experience in hospice, home health, ICU, geriatric, or oncology.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
