Job Summary

The Home Health LPN provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. This role is ideal for a licensed practical nurse who is passionate about delivering high-quality patient care in a home setting.

Responsibilities
  • Provides skilled nursing care to patients in their homes, following the plan of treatment and nursing care plan.
  • Evaluates the significance of assessment findings and communicates any pertinent information about the patient's clinical status and ongoing needs to others in a timely fashion.
  • Assesses and reassesses pain, utilizing appropriate pain management techniques and educating the patient and family regarding pain management.
  • Demonstrates knowledge of medications and their correct administration based on the patient's age and clinical position.
  • Follows the seven medication rights and reduces the potential for medication errors.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Functions as a team member, demonstrating collaboration with and responsibility to the patient, physician, and interdisciplinary team.
  • Adheres to all HHA, payroll, billing, and documentation policies and procedures.
  • Participates in HHA's other mandated activities, as requested by the Clinical Supervisor or Patient Care Services Director, including in-services and the Upper Respiratory (UR)/Perfusion Index (PI) committee.
  • Performs patient care responsibilities considering the needs of the patient to the standard of care for the patient's age.
  • Articulates the HHA grievance/concerns procedure and the HHA adverse event/incident reporting procedure.
  • Articulates the organization's emergency management plan.
  • Assists the physician and registered nurse to perform specialized procedures.
  • Works as part of the interdisciplinary team.
  • Completes patient documentation on time.
  • Attends in-services and staff meetings as needed.
  • Completes assigned training on time.
  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Performs other duties as assigned.
Qualifications
  • Graduate of an accredited nursing program.
  • Current state licensure as a LPN.
  • Experience as an LPN commensurate with one of the following:
  • One year of experience in an acute care setting within the last 24 months;
  • Six months of home care experience within the last 12 months.
  • Prior home healthcare experience is preferred.
  • Valid state driver's license and reliable automobile, current automobile insurance, and being willing to operate a personal car necessitated by the nature of the job.
  • Current TB testing.
  • Current CPR card.
  • No findings relating to client abuse, neglect, or misappropriation of client property.
