Job Summary

Select Home Health is a leading provider of home healthcare services in central Indiana. We are seeking a skilled Clinical Home Health Intake Coordinator to join our team.

The successful candidate will be responsible for completing the full intake process, including inputting patient information into our system, confirming physician details, and verifying Face to Face visits.

Key Responsibilities

  • Efficiently process incoming referrals for all branches.
  • Understand and comply with regulations and requirements for admission to Home Health.
  • Communicate effectively with physicians, patients, and sales and marketing staff.
  • Coordinate and confirm valid Face to Face visits and attestation forms.
  • Promote compliance with incoming documentation and monitor admission percentages.
  • Run and maintain intake reports within our system.
  • Maintain a referral tracking sheet and demonstrate a desire to improve processes.
  • Perform job duties in compliance with agency policies and procedures.

Requirements

  • Current LPN licensure in the state of Indiana.
  • Business and professional setting experience.
  • Healthcare experience preferred.
  • Excellent written and verbal communication skills.
  • Ability to speak, read, write, and comprehend English.
  • Ability to handle multiple tasks simultaneously and meet deadlines.
