Healthcare Navigator Specialist

We are seeking a highly skilled Healthcare Navigator Specialist to join our team at Apex Therapeutic Services. As a key member of our healthcare services team, you will play a vital role in conducting initial assessments of new patients, developing effective care plans, and ensuring seamless access to necessary healthcare services.
Key Responsibilities:
- Conduct thorough assessments of new patients to identify their unique healthcare needs and develop personalized care plans.
- Maintain strong relationships with healthcare professionals and community organizations to ensure comprehensive care coordination.
- Coordinate patient care plans, including scheduling medical appointments, arranging diagnostic tests, and managing prescription medications.
- Utilize electronic medical records (EMRs) to document patient information and track progress toward care goals.
Requirements:
- Bachelor's degree in social work or a related healthcare field.
- Minimum 2 years of experience in healthcare coordination, patient navigation, or a related field.
- Knowledge of healthcare delivery systems, protocols, and regulations.
- Excellent communication and interpersonal skills.
- Strong organizational and time management capabilities.
- Familiarity with EMR systems and other healthcare software programs.
- Starting Hourly Rate: $18.00 per hour
