Job Summary

We are seeking a highly skilled Registered Nurse to join our Care Navigation team at Toledo Hospital. As a Care Navigator RN, you will play a critical role in ensuring the seamless transition of patients through the healthcare system, providing exceptional care and support to patients and their families.

Key Responsibilities
  • Conduct initial assessments with patients and caregivers to identify patient decision makers and develop comprehensive care plans.
  • Collaborate with interdisciplinary teams to coordinate patient care, including physicians, nurses, social workers, and other healthcare professionals.
  • Develop and implement discharge plans, including contingency plans, to ensure patients receive the necessary care and support after hospitalization.
  • Assess patients to determine their ability for self-care and identify those at risk for post-discharge adverse health consequences.
  • Provide patient-centered education and support to patients and their families, including disease process education, medication management, and post-discharge care instructions.
  • Coordinate patient access to necessary services, including community and public health resources.
  • Ensure patients have an identified primary care physician and follow-up appointment scheduled prior to discharge.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
Requirements
  • Current, active, and unrestricted licensure as a Registered Nurse in the state of practice.
  • 5 years of applicable nursing experience.
  • Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills.
  • Ability to work in a self-directed environment with attention to detail and follow-through.
  • Ability to establish priorities and respond to inquiries.
  • Ability to manage multiple tasks and prioritize levels of importance.
Preferred Qualifications
  • MSN or higher degree.
  • Applicable experience in patient advocacy, care management, and knowledge of hospital and community resources.
  • Specialty certification in care management (CCM, RN-BC, ACM, or similar).
