Nurse Leadership and Patient Care Coordinator

Salary: $85,000 - $110,000 per year

We are seeking a highly skilled Nurse Leadership and Patient Care Coordinator to join our team at Houston Methodist Cypress Hospital. As a key member of our nursing staff, you will be responsible for providing exceptional patient care and leading by example to promote a positive and supportive work environment.

Job Description

About the Role:

  • We are seeking a registered nurse with at least 24 months of clinical experience in a hospital setting.
  • You will be responsible for providing high-quality patient care, including assessing patients, developing and implementing care plans, and educating patients and families on health conditions and treatments.
  • As a leader on our team, you will mentor and coach other nurses, provide guidance and support, and contribute to the development of departmental policies and procedures.

About You:

  • You have a Bachelor of Science degree in Nursing from an accredited school of nursing.
  • You are licensed as a registered nurse in the state of Texas or compact state licensure within 60 days.
  • You hold Basic Life Support (BLS) certification from the American Heart Association.
  • You have strong communication and leadership skills, with the ability to work effectively with patients, families, physicians, and other healthcare professionals.
  • You are able to prioritize tasks, manage multiple responsibilities, and maintain accuracy and attention to detail in a fast-paced environment.
Key Responsibilities
  1. Patient Care:
    1. Assess patients' physical and emotional needs, develop and implement individualized care plans, and evaluate patient outcomes.
    2. Educate patients and families on health conditions, treatments, and self-care techniques.
    3. Collaborate with interdisciplinary teams to ensure comprehensive patient care.
  2. Leadership and Education:
    1. Mentor and coach other nurses, providing guidance and support to help them grow in their roles.
    2. Contribute to the development of departmental policies and procedures, ensuring they align with hospital standards and regulatory requirements.
    3. Participate in continuing education programs to stay current on best practices and new developments in nursing care.
  3. Communication and Teamwork:
    1. Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care transitions.
    2. Collaborate with colleagues to achieve shared goals and objectives, promoting a positive and respectful work environment.
