PACE Southeast Michigan is seeking a skilled Nurse Care Manager to join our team. As a key member of our multidisciplinary care team, the Nurse Care Manager will work closely with participants, family members, and healthcare professionals to provide high-quality, patient-centered care.

About the Role

The Nurse Care Manager will be responsible for assessing participant needs, developing individualized care plans, and implementing evidence-based interventions to promote health, functional ability, and overall well-being. This role requires excellent communication, interpersonal, and leadership skills, as well as the ability to work collaboratively with interdisciplinary teams.

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, in addition to a comprehensive benefits package that includes medical, dental, and vision insurance, 401(k) matching, and paid time off.

Responsibilities
  1. Assess participants' physical, emotional, and social needs, and develop individualized care plans to address these needs.
  2. Collaborate with participants, family members, and healthcare professionals to implement care plans and promote health outcomes.
  3. Provide education and support to participants and their families to empower them to take an active role in their care.
  4. Lead and direct licensed and non-licensed staff in the delivery of care, ensuring adherence to best practices and regulatory requirements.
  5. Evaluate participant outcomes and progress toward care plan goals, making adjustments as needed to ensure optimal results.
Requirements
  • RN licensure in Michigan required; BSN preferred.
  • Minimum one year of experience working with frail or elderly populations.
  • Excellent communication, interpersonal, and leadership skills.
  • Ability to work collaboratively with interdisciplinary teams.
  • Strong problem-solving and critical thinking skills.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
Working Conditions

This is a full-time position that requires frequent travel within the PACE Southeast Michigan catchment area. The successful candidate must have a valid driver's license, reliable transportation, and be able to navigate varied environmental conditions, including extreme temperatures.
