Care Coordinator for Senior Communities

About the Role:
We are seeking a dedicated Care Coordinator to join our team in [location]. As a Medication Care Manager, you will play a vital role in providing high-quality care and services to our residents. This is a full-time opportunity with a competitive salary range of $65,000 - $85,000 per year.
Responsibilities:
The successful candidate will be responsible for administering medications and treatments in a safe and organized manner. Key duties include:
• Providing medication administration services to residents in accordance with their Individualized Service Plans (ISPs).
• Ensuring accurate and timely documentation of medication administration and resident care activities.
• Collaborating with the Resident Care Director (RCD) and Wellness Nurse to review and update medication records as necessary.
• Participating in daily Cross Over meetings to discuss resident care plans and address any concerns.
• Maintaining confidentiality of all resident information and adhering to Sunrise's policies and procedures.
Requirements:
To be successful in this role, you will need:
• High school diploma or equivalent required; certification in medication administration may be preferred.
• Excellent communication and interpersonal skills, with the ability to work effectively with residents, family members, and healthcare professionals.
• Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
• Ability to maintain accurate records and documentation, with attention to detail and a commitment to quality care.
• Familiarity with Sunrise's policies and procedures, including those related to medication administration and resident care.
• Valid driver's license and reliable transportation (as applicable).
