Key Responsibilities:
  • Deliver direct patient care under the guidance of a Registered Nurse and in accordance with the Aide Plan of Care (POC).
  • Assist patients with self-administered medications, ensuring their safety and well-being.
  • Take accurate temperature, pulse, and blood pressure measurements, recognizing and reporting any abnormal results to your supervisor.
  • Support patients in maintaining good personal hygiene, including bathing, grooming, skin care, and other essential activities.
  • Assist with feeding patients, promoting healthy nutrition and hydration practices.
  • Help patients with mobility, exercises, positioning, transfers, and ambulation, as outlined in the Plan of Care and CenterWell Home Health policy.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care and accurate record-keeping.
  • Practice infection control principles, maintaining a clean, safe, and comfortable environment for patients.
  • Contribute to household duties, such as light laundry, bed changing, and meal preparation, as needed.
Requirements:
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
  • Meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are necessary.
  • Be in good standing on the HHA Registry (if applicable) and have completed the HHA/CNA course to work for a Medicare-certified agency.
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
