Brighton Hospice is seeking a compassionate and skilled Social Worker to join our team. As a Social Worker, you will play a vital role in providing social services to patients and their families, and collaborating with our clinical team to ensure the best possible care.

Key Responsibilities
  • Provide short-term individual counseling and crisis intervention to patients and their families
  • Assist in providing information and preparation of advance directives
  • Support patients and families in funeral planning and transfer of responsibilities regarding fiscal, legal, and health care decisions
  • Identify support systems available to reduce stress and facilitate coping with end-of-life care
  • Utilize community resources to support patients and families
  • Perform additional duties as needed to support the care team

Qualifications

We are seeking a candidate with:

  • An active license in the state of employment
  • A valid driver's license, auto insurance, and reliable transportation
  • One year of hospice-related experience (preferred)
  • A Master's degree in social work from a Council on Social Work Education-accredited program (strongly preferred)
  • A passion for providing care to patients and their families during and after the dying process
  • A desire to work in a team environment
