About the Role:

We are seeking a skilled Physical Therapist to join our Home Health team at SSM Health. As a Physical Therapist, you will play a vital role in evaluating and conducting medically prescribed physical therapy treatment programs to treat movement dysfunction and pain resulting from injury, disease, disability, or other health-related conditions.

Key Responsibilities:

  • Evaluate and Assess: Perform initial and on-going assessments of patient's condition to identify areas of improvement and develop a personalized treatment plan.
  • Develop and Implement Treatment Plans: Establish, revise, and evaluate a plan of care that is tailored to the patient's specific needs and goals.
  • Provide Skilled Therapy Interventions: Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
  • Education and Training: Provide education to patients and families on proper techniques and exercises to promote optimal recovery and independence.
  • Continuity of Care: Participate in discharge planning with patients, families, and other healthcare providers to ensure a smooth transition to the next level of care.
  • Leadership and Supervision: Oversee the activities of physical therapy assistants, students, and other support personnel to ensure high-quality patient care.
  • Documentation and Communication: Maintain accurate and detailed documentation of patient progress and communicate effectively with healthcare teams to ensure seamless care coordination.

Requirements:

  • Education: Bachelor's, Master's, or Doctorate degree in Physical Therapy or related field required.
  • License and Certification: Current licensure as a Physical Therapist in the state of practice and Basic Life Support (BLS) certification required.
  • Experience: No experience required, but prior experience in home health or physical therapy preferred.

Work Environment:

This is a full-time position with a scheduled weekly hours of 40. The work environment is fast-paced and dynamic, with a focus on providing high-quality patient care in a home health setting.
