Speech Language Pathologist, Home Health Specialist Opportunity

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist to join our team in providing home health visits throughout Windsor County, VT. As a member of our multidisciplinary healthcare team, you will work closely with clients, families, and other healthcare professionals to provide rehabilitative care and skilled nursing services.

Responsibilities:

  • Make home visits to clients in designated geographic territories.
  • Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
  • Develop and implement treatment plans based on evaluations, physician's orders, and recommended goals.
  • Continually assess and revise the speech therapy care plan, participating in multidisciplinary care planning as needed.
  • Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations related to patient care management and services provided.

Qualifications:

  • A current license as a Speech Language Pathologist in VT.
  • Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or have equivalent educational requirements and work experience.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
