Job Summary

This position will be a member of a team of behavioral health providers in the Center of LGBTQ Health Equity at Chase Brexton Health Care, providing direct behavioral health services in support of integrated primary care. The successful candidate will provide prevention, intervention, and recovery from behavioral health issues and concerns, as well as assessments and letters for gender confirmation surgeries within the primary care setting.

Key Responsibilities
  • Conduct brief, evidence-based screenings and time-limited assessments
  • Provide GCS assessments and surgical letters as needed, with training provided
  • Apply knowledge of evidence-based practice to individuals, couples, groups, and families, including adults, children, and adolescents
  • Assess discharge readiness throughout treatment and provide emergency assessments of patients in crisis
Problem Solving and Decision Making
  • Create and implement integrated treatment plans, ensuring access to organizational and external services within allotted time frames
  • Evaluate treatment progress and modify planning as indicated, even in the absence of established outcome measures
Communication
  • Provide consultation to medical providers, other CBHC staff, and service providers to facilitate a coordinated continuum of care
  • Participate in staff meetings and collaborative teamwork, ensuring timely and well-integrated communication
  • Participate in system change and promote change as identified within Chase Brexton Health Care
Patient Focus
  • Provide referrals for internal and external services, with a focus on the LGBTQ community
  • Organize and maintain educational and referral materials for the Center for LGBTQ Health Equity
  • Demonstrate ability to engage patients and families who are not engaged in services through outreach and follow-up
Compliance, Policy, and Procedure
  • Document all behavioral health services as directed by CBHC policy, State requirements, and professional standards
  • Adhere to departmental productivity standards and policy and procedures
  • Assess for risk using a standardized measure and complete all compliance assignments
Workplace Computers and Equipment
  • Utilize Electronic Health Record to support and improve integrated healthcare
Willingness to Learn
  • Meet mandatory continuing education requirements to maintain licensure/certification
  • Perform other duties as assigned by supervisor
Skills and Abilities
  • Ability to provide services relevant to the culture of patients
  • Ability to identify and address disparities in healthcare access and quality
  • Ability to adapt services to cultural norms and promote inclusion among peers
  • Clinical experience with LGBTQ patients and community
  • Familiarity with open access model to care in community mental health and healthcare settings
Education and/or Experience
  • Minimum Master's Degree in Social Work
  • LCSW-C licensure in the state of Maryland
  • Experience in outpatient mental health services and LGBTQ behavioral care, ideally in a primary care environment
