Job Title: Nurse Transition Leader

At Otterbein SeniorLife, we are seeking a skilled and experienced Nurse Transition Leader to join our team. As a key member of our leadership team, you will be responsible for providing leadership and supervision to licensed healthcare areas and staff at assigned locations.

Key Responsibilities:
  • Obtain daily reports from nurses and conduct effective daily rounds
  • Discuss observations and concerns about clinical compliance with nurses or care coordinators
  • Monitor compliance of collection and submission of MDS data
  • Review and ensure staff compliance with in-service requirements and self-development programs
  • Monitor Quality Measures and coordinate Quality Improvement initiatives
  • Monitor clinical budgets and act as a fiscal steward
  • Complete incident reporting and follow-up
  • Act as a liaison with Medical Directors and complete monthly reports and quarterly QA initiatives
  • Effectively supervise staff in all areas of employment in partnership with Human Resources
  • Act as a liaison with healthcare vendors
  • Work on the floor when needed
  • Act as a liaison with families when appropriate
Requirements:
  • Education: Bachelor of Science in Nursing (BSN)
  • Licensure/Certification: Current Ohio RN license; RN license in Indiana a plus
  • Experience: Minimum 3 years of Director of Nursing experience in long-term care/nursing home environment required
