Nurse Leader

We are seeking a skilled and compassionate Registered Nurse to join our team as a leader in the Transitional Care Unit at Kaiser Permanente's Zion Medical Center. As a key member of our healthcare team, you will provide high-quality patient care, utilizing the nursing process to ensure optimal outcomes.
Responsibilities- Develop and implement individualized plans of care that reflect assessment, planning, implementing, and evaluating patient outcomes.
- Collaborate with multidisciplinary teams to ensure comprehensive care and effective communication.
- Provide education and support to patients and families, promoting health literacy and self-management.
- Participate in performance improvement activities, identifying opportunities for systems improvement and contributing to departmental goals.
- Adhere to regulatory requirements, policies, and procedures, ensuring a safe and efficient work environment.
- One year of recent experience in a Critical Care, DOU, or SDU setting, or successful completion of a KP Critical Care course.
- Registered Nurse License (California) and Basic Life Support certification.
- Advanced Cardiac Life Support certification and Basic Arrhythmia course completion.
