Occupational Therapist Home Health Full Time

CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will play a vital role in helping patients recover from illness, injury, or surgery and regain their independence.
Key Responsibilities- Assess patients' daily living and work-related skills and develop personalized rehabilitation plans.
- Administer and interpret diagnostic tests to evaluate patients' level of function and assist physicians in developing treatment plans.
- Collaborate with healthcare teams to develop and implement care plans that meet patients' unique needs.
- Provide therapeutic treatment and instruction to patients to improve strength, coordination, and range of motion.
- Review and expand rehabilitation plans to ensure patients achieve their behavioral goals.
- Consult with other healthcare professionals to develop vocationally oriented plans and provide education to patients and their families.
- Report patient progress to physicians, rehabilitation staff, and other healthcare professionals.
- Master's degree from an accredited Occupational Therapy program.
- Minimum of six months of occupational therapy experience, with home health experience preferred.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
