Key Responsibilities:
  • Conduct initial and ongoing assessments to evaluate patients' mobility, coordination, strength, and functional abilities.
  • Develop and implement individualized plans of care to address patients' occupational therapy needs.
  • Collaborate with interdisciplinary team members to ensure comprehensive care and optimal patient outcomes.
  • Document patient progress and maintain accurate records in accordance with company policies and procedures.
  • Participate in discharge planning and education to ensure a smooth transition for patients and their caregivers.
Requirements:
  • Master's degree in Occupational Therapy from an accredited university/program.
  • Current, unrestricted state license as an Occupational Therapist in the state of practice.
  • Valid CPR certification.
Preferred Qualifications:
  • Strong organizational and communication skills.
  • Attention to detail and ability to manage multiple priorities.
  • Proven experience in home health therapy and OASIS documentation.
